Arrival/Registration Information

► If you are a new patient, please arrive 20 to 30 minutes early so that all appropriate paperwork can be completed.

► Be certain to bring your insurance card, even if you have brought it in the past.

► Make every attempt to wear metal-free clothing, and remember, that it may be necessary to change into an examination gown for certain procedures.

► If there is any possibility that you may be pregnant, please notify our staff at the time of scheduling. Also, please inform the technologist performing the exam that you may be pregnant.

► If you have had a prior allergic reaction to iodinated contrast (x-ray dye), such as hives or difficulty breathing, please let us know at the time of scheduling.

► If you have pertinent x-rays or prior imaging studies of a similar type, please bring them.

 

REGISTRATION FAQ

What do I bring with me to my appointment?

► Your insurance card, including your ID number and group number. We will make a photo copy of your insurance card.

► For worker’s compensation claims and automobile injuries, bring your claim number and the date of the injury. Please inform your doctor’s office if your scan will be covered under worker’s compensation.

► Please bring any outside films that might pertain to the scan being done. This will help the radiologist in reading your scan.

Do I need to be pre-authorized by my insurance company before my scan?
As a courtesy, Jackson Imaging Center will try to get the pre-authorization on all CTs and MRIs. It is ultimately the patient’s responsibility to verify that authorization has been obtained prior to the scan.

Can Jackson Imaging Center file primary and secondary claims?
Yes, we can!

What if I don’t have insurance?
Jackson Imaging Center will give you an estimated cost of your exam. We will place you in contact with our billing department to help you set up a payment plan.

Do I have to pay at the time of the service?
We would appreciate at least a partial payment. However, we understand that at times this can be difficult. Our billing office can help you make payment arrangements.

How can I pay for my scan?
We accept cash, checks, Visa and MasterCard.

Can I send results to more than one physician?
Yes.
